Admission/Referrals

Women are admitted to our program through a referral process. Every referral is reviewed on a case-by-case basis with required form submissions and approval via a clinical assessment.

Admissions Criteria

  • Must be at least 21 years of age
  • Be able to pay program fees – Level I Housing $100; Level 2 Housing: Single Room $450 /mo.
    Double Room $350/mo.
  • Must have 28-30 days clean/sober or have completed a Detox Program (for 1st Level Housing); 9 months or more (2nd Level Housing)
  • Must be referred to the program by a counselor or case worker
  • Self-referrals will be entertained by a case-by-case situation (Must be able to demonstrate sobriety/clean time)
  • Individuals are be responsible for their behavior
  • Attend 12 steps meetings or engage in other Pathways of Recovery
  • Meet with clinical staff
  • Pay a program fee

Procedure

  • Refer client using Admissions Documents
  • Provide Biopsychosocial and discharge information
  • Provide any and all pertinent medical information
  • Send all requested documentation via email to intake@lightoftruthcenter.org or fax to
    (866-910-5074)
  • Schedule a telephone intake and a tour of the facilities
  • After the intake and tour is complete, clients will be notified within 48 hrs. of acceptance/non-acceptance
  • Move in date and instructions will be given upon acceptance

Download LTC Referral form and fill out. Once completed, click on the “Submit Your Documents” button, fill out required info, upload your document(s) and submit. Upon receipt, an ancknowledgement will be sent.

Exclusionary Information

If a potential consumer is found ineligible for services, the information regarding the reasons will be disclosed and referral information will be provided. Those involved in the referral of the person will be informed as to the reasons.

Potential participants who present the following symptoms or behaviors will not be admitted for services but will be referred to a more targeted provider. Those criteria include:

  • A conflict of interest or ethical concern in providing services.
  • Exhibiting a level of dysfunction that requires more intensive and/or restrictive level of care than the organization can provide.
  • Persons with conduct problems that feature physical aggression with severe safety risks.
  • Persons with cognitive or developmental differences that preclude them from benefiting from counseling services.
  • Persons needing or requesting medication management services only.
  • Applicants must be able to communicate their desire for change, not have significant medical or mental issues, and if unable to feed or clothe themselves, applicants are ineligible for admission to this level of care.
  • Medically and/or cognitively fragile applicants who are unable to function or are unable to benefit from any of LTC’s programs.

A referral to an appropriate level of care or alternative services will be provided to an applicant who is found to be clinically inappropriate for any of LTC’s treatment programs.

When an applicant is found to be clinically inappropriate for housing or treatment at the Light of Truth Center, Inc. the referral source, participant, and family member (if applicable) are notified via phone, email, or letter.
